His date and place of birth and his parents' names are given in his Indiana birth certificate.

South Bend News-Times, Wed., Nov. 17, 1915, p. 3:
"Charles Joseph Voorde, the infant son of Mr. and Mrs. David Voorde, died Wednesday morning after an illness of four days. He was born in South Bend on Sept. 14, 1914. Besides the parents, he is survived by four brothers and two sisters. The funeral will be held Thursday afternoon from St. Mary's church. Rev. Father Scherer officiating. Burial will be in Cedar Grove cemetery." His date and place of death also are given in his Indiana death certificate for which the informant was Joseph Voorde, known from other records to be his brother. His understanding was that Charles had been born Sep. 14, 1914 in South Bend to David and Anna Voorde.
