SOUTH BEND - Frank T. Polack, 94, of South Bend, IN, passed away at 5:47 p.m. Saturday, April 23, 2016 at Healthwin Specialized Care in South Bend, IN. He was born on September 21, 1921 to the late Stanley and Mary (Kozlowski) Polack in Mt. Carmel, PA and has lived in South Bend since 1946. Along with his parents, Frank was preceded in death by three sisters, Mary Bianchi, Dorothy Hanna and Irene Kashmer; and three brothers, Edward, Stanley and Raymond Polack. Frank was self-employed for 25 years as a General Insurance Agent in South Bend. Prior to that, he worked at Studebakers. On July 1, 1946, he married Justine H. “Tina” Niemier at St. Casimir Catholic Church in South Bend, IN. She passed away on April 6, 2008. Frank is survived by his five daughters, Jacinta Polack of South Bend, IN, Jocelyn (Wayne) Polack Raybuck of Mishawaka, IN, Tina Marie (Patrick) Deka of South Bend, IN, Jan Marie (Greg) Hyduk of South Bend, IN and Andrea (Fredrick) Bedford of Granger, IN; two sons, Marc (Patricia) Polack of Fort Gratiot, MI and Christian (Veronica) Polack of South Bend, IN; 13 grandchildren; six great-grandchildren; and a sister, Lorraine Kobi Daphnee of Alabama. Frank was a LTJG in the U.S. Navy. He was a World War II veteran, where he served on the USS Boxer and the USS Valley Forge from 1942-1945. He received a Victory Medal, American Theatre Medal, Asiatic Pacific Medal, and a China Service Medal while serving. He successfully completed the course of the United States Naval Reserve Midshipmen's School at the University of Notre Dame ROTC in 1945. He also attended V-12 training; Tufts College in Medford, MA; George Washington University in Washington, D.C.; and Gen. Line School in Newport, RI.Frank was a parishioner of Sacred Heart Parish, Notre Dame, IN. His love of his garden, nature, and family are endless and will always be remembered, cherished, and sadly missed. There will be no visitation. A Mass of Christian Burial will be held at 3:00 p.m. Wednesday, April 27, 2016 at the Basilica of the Sacred Heart at Notre Dame, with Rev. Peter Rocca, C.S.C. officiating. Military Services will follow in Southlawn Cemetery. The family wishes to express their heartfelt gratitude to the entire Healthwin family the past three years, especially all the staff in 2 West. We appreciate all you do and will always remember your kindness and devotion.
