His date and place of death are given in his Indiana death certificate for which Rev. Bernard Niemier was the informant. His understanding was that Rev. Fr. Casimer J. Witucki had been born Aug. 6, 1900 in South Bend, Indiana to Andrew & Agnes (Kujawa) Witucki.
